How to Get Started – Your Path to Support
If you or a family member is living with 'end stage' kidney disease, we're here to help. Our application process is straightforward, and your social worker is ready to guide you through every step.
Basic Requirements
Diagnosis: You must have an end stage kidney disease diagnosis from a healthcare provider.
Location: You must live or receive treatment in our 12-county service area (9 TN counties, 3 GA counties).
Documentation: You'll need to provide proof of total household income and be within 200% of Federal Poverty guidelines.
Service Policy
We will extend equal opportunity to all individuals without regard for race, religion, color, national origin, age, disability, sexual orientation, gender, identity, handicaps, or veterans status.
No patient is entitled to any service or specific dollar amount. All applications considered on a case by case basis.

How to Apply
Getting help is easy. Follow these three simple steps:
Contact Your Social Worker
Contact your clinic social worker for assistance. Call us at 423-265-4397 or email info@kidneyfoundation.com if you need assistance.
Complete Your Application
Provide your social worker with basic information, household expenses and proof of household income, and invoice for service requested.
Receive Your Assistance
Most applications are processed quickly. Once your application is approved, we'll work with your social worker directly to provide the help you need.
What Documents Do I Need?
To help us process your application quickly, please gather these documents:
Proof of Household Income
The following forms are acceptable: Social Security statement, pay stub, child support, food stamps approval, WIC approval, or current bank statement.
Current Bill/Invoice
A utility bill, lease agreement, or other document showing the total amount due. Disconnect notices are not accepted.
Other Vital Information
Any information helpful to paying bill provided such as, social security number, account number and/or log ins, accurate mailing address, etc.
